Location: Port Charlotte, Florida
Shift: Full Time
Salary: $55 – $57 per hour

About the Role

Sun Harbor is seeking a dedicated and compassionate Physical Therapist to join our dynamic rehabilitation team. In this vital role, you’ll help residents regain strength, mobility, and independence through personalized care plans and hands-on therapeutic support.

Responsibilities:

• Evaluate residents and develop individualized treatment goals and physical therapy plans.

• Implement and document therapeutic interventions focused on gait, ambulation, muscle strength, coordination, endurance, mobility, posture, sensory awareness, balance, and orientation.

• Design, fit, and provide training on adaptive devices, including orthotics and prosthetics.

• Select and apply physical modalities suited to each resident’s diagnosis, physical capacity, and mobility needs.

• Collaborate closely with the rehabilitation team to determine optimal therapy regimens.

• Instruct residents in mobility activities, monitor progress, and adjust treatment plans accordingly.

• Participate in resident care and staffing meetings, contributing clinical expertise.

• Supervise supportive personnel to ensure compliance with state and professional standards.

• Conduct training programs and assist in educating medical, nursing, and rehabilitation staff on physical therapy practices.

• Manage ordering and maintenance of therapy supplies and equipment, ensuring proper cleanliness and reporting repairs as needed.

Requirements:

• Minimum of 5 years of experience as a Physical Therapist.

• Current, unencumbered Florida Physical Therapist license.

• Long-term care experience required.

• Proficiency in computer systems: Net Health and PointClickCare.
