Physical Therapist (PT)

Position Overview The Physical Therapist (PT) provides direct rehabilitation care in physical therapy to patients in a skilled nursing facility setting. Responsibilities Developing and implementing personalized treatment plans based on physician referrals Assessing and adjusting goals to promote patient progress and functional improvements Supervising and evaluating care provided by PT aides, PTAs, and students Documenting patient progress accurately and timely using therapy software Collaborating with interdisciplinary team members to ensure comprehensive patient care Maintaining professional relationships with patients, families, and staff Adhering to safety, confidentiality, and compliance standards Participating in community health initiatives as appropriate Qualifications & Skills Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT); new grads are welcome Current state license in good standing CPR certification (obtained upon hire or during orientation) Proficiency in Microsoft Office and email communication Strong interpersonal, decision-making, and body mechanics skills Knowledge of post-acute care laws, regulations, and procedures Key Duties Follow treatment plans, set measurable goals, supervise therapy staff, maintain documentation, provide excellent patient service, and participate in safety and emergency procedures. #J-18808-Ljbffr MEP Health
