Job Description Nestled between Burbank and Pasadena, Adventist Health Glendale is one of the area's leading healthcare providers. Job Summary Acts as a patient advocate, guiding patients through the oncology care system and establishing contact with newly diagnosed cancer patients, their families, or caregivers. Understands the clinical care options for patients and directs them to healthcare services within the organization, outside facilities, and community resources for timely treatment and survivorship. Actively identifies and addresses patient‑specific barriers to care, connects patients with resources and support services, and supports the physician and interdisciplinary team in enhancing patient satisfaction and clinical outcomes. Implements day‑to‑day care‑management interventions for identified clinic patients. Job Requirements Bachelor's Degree in Nursing (BSN): Preferred Oncology nursing experience: Preferred Experience in healthcare screening or cancer control activities: Preferred Licenses/Certifications Registered Nurse (RN) licensure in the state of practice: Required Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification: Preferred Oncology Certified Nurse (OCN): Preferred Essential Functions Assists patients and families in identifying and accessing appropriate institutional and community resources, implements care interventions and follow‑up to ensure timely care and prevent delays. Makes initial and ongoing assessments of patient needs (clinical, emotional and social) and makes appropriate recommendations or referrals for care. Collaborates with patients, families and healthcare providers to provide patient care in a safe, healing, humane, and caring environment, providing health information as needed. Documents coordinator and patient interactions in electronic or hard copy chart in a manner consistent with hospital standards, and creates reports from patient data such as volumes and outcomes. Assists research nurses and oncologists with accruing eligible patients to clinical trials, performing duties relevant to clinical trials including obtaining informed consent, patient instruction, and documentation per protocol.
