About the Opportunity

We are seeking a skilled and compassionate Speech-Language Pathologist to join a dedicated healthcare team committed to delivering high-quality, resident-centered care. This is an excellent opportunity for an experienced SLP or a Clinical Fellow looking to grow within a supportive skilled nursing environment.

Key Responsibilities

Conduct comprehensive evaluations of residents' speech, language, cognitive, and swallowing needs and develop individualized, goal-directed treatment plans.
Design and implement therapeutic activities that promote resident independence in communication and daily functional tasks.
Monitor and assess resident progress on an ongoing basis, adjust treatment plans as clinically indicated, and prepare detailed progress reports.
Provide skilled rehabilitation support to residents recovering from injury, illness, or other conditions affecting communication and swallowing.
Maintain accurate, complete, and timely documentation of resident care, progress notes, and department records in accordance with regulatory standards.
Uphold strict patient confidentiality and ensure the secure handling of all protected health information.
Maintain a clean, safe, and supportive environment for residents and staff at all times.
Collaborate closely with physicians, nursing staff, and the interdisciplinary care team to ensure coordinated, comprehensive resident care.
