Youth Mental Health Case Manager

CarePath Behavioral Health in Alabama seeks a Child and Adolescent Care Coordinator (case manager) to provide mental health case management for children and teens within our program. The role involves completing needs assessments, developing case plans, coordinating with families, schools, and community services, and helping to stabilize clients in the community while coordinating with In Home and Outpatient teams.
