Case Manager III (FT, Back End Days)

Our values shape our decisions, define our culture, and foster continuous learning and growth. $10,000 commitment bonus for full-time Earn More with Shift Differentials +$2/hr -> Monday-Thursday Nights +$3/hr -> Friday-Sunday Nights +$2/hr -> Saturday-Sunday Days What You'll Do: Facilitate admission intakes, treatment planning, discharge planning, obtaining collateral, clinical interventions, crisis management, assists in the legal process, and initiates personalized care for individuals who are in crisis and stabilizing from crisis. Work collaboratively with the interdisciplinary treatment team to coordinate care and discharge planning for individuals, their family members, clinics, local providers, and other referral sources. Conduct a thorough, accurate, clinically sound Psychosocial Assessment with the individual in a timely manner. Review and sign off on Psychosocial Assessments completed by certified AAC or associate level staff members, if fully licensed. Educate and inform individuals of consents for treatment and authorizations for use and disclosures. Participate regularly in Interdisciplinary Treatment Team meetings. Collaborate with the Treatment Team to develop and implement an individualized person-centered Treatment Plan appropriate for brief intervention. Gather collateral from the individual and involved parties to assess treatment and discharge needs. Maintain a working knowledge of the Involuntary Treatment Act (ITA) and Evaluation and Treatment (E&T) Involuntary Commitment process; monitor the petition process to ensure timeliness and completeness; attend E&T hearings when subpoenaed. Coordinate care and provide referrals to ensure services are aligned with facility staff and external providers. Identify and address gaps in service needs for participating parties; recommend solutions to meet individuals’ needs upon discharge. Engage, monitor, and communicate with the individual to maintain safety while meeting needs. Redirect individual behavior while maintaining a safe milieu when necessary; document all services and activities appropriately. Provide internal education regarding DBT, trauma-informed care, harm reduction, and service plan documentation to enhance staff capacity. Provide case management services to promote recovery and community reintegration. Maintain clinic records, including documentation of activities and obtain signatures from individuals regarding treatment. Prepare clinical reports and individualized service plans utilizing agency electronic clinical documentation software; ensure documentation complies with best practices and regulatory requirements. Maintain knowledge of Washington State behavioral health system and potential funding sources. Participate in individual and group supervision; engage in ongoing education and demonstrate competency via assessments. Perform all other duties as assigned. What You'll Bring: Master's degree in behavioral health, social work, counseling, psychology, or related discipline. Unrestricted license as a Licensed Mental Health Professional in Washington State or an associate license. Three years of experience in mental health or substance abuse treatment. Knowledge of Washington State behavioral health system of care and resources. Mandatory vaccination policy compliance. Completion of all credentialing requirements within required time period (NPI, Medallion enrollment, Medicare enrollment, etc.). Preferred qualifications (optional): Experience working with children, youth, families, and adults with behavioral health issues. Knowledge of and experience with the Washington State involuntary treatment process.
