Nurse Practitioner

Overview ChenMed is a physician‑led primary care organization that serves Medicare Advantage seniors. The Primary Care Physician (PCP) will deliver clinical care and drive outcomes for a patient panel of up to 450 patients. The role blends direct patient care with leadership responsibilities to promote a culture of accountability and quality. Essential Job Duties / Responsibilities Independently provide care for patients with acute and chronic illnesses common in older adults. Take full accountability for patient care and outcomes; seek consultations from specialists when needed while remaining ultimately responsible for patient management. Engage with hospitalists when a patient is hospitalized, regardless of hospitalist affiliation. Assess, diagnose, treat, manage, educate, promote health, coordinate care, and document for patients with acute or complex chronic needs. Lead a care team composed of a medical assistant, care facilitator, and care coordinator for office‑based patients. For patients who cannot visit the office—whether in hospital, skilled nursing facility, long‑term care, or homebound—collaborate with transitional‑care teams, case managers, and other resources as appropriate to the market. Actively participate in center management, cover for other providers when needed, and assist in recruiting patients and additional providers. Perform other duties assigned or modified by a manager. Knowledge, Skills & Abilities Availability and accessibility to patients to build trust; respond to walk‑ins, phone calls, and messages promptly. Proficiency with Microsoft Office Suite and other common business software. Willingness and ability to travel locally as needed, nationally for initial training (2 weeks), and occasionally regionally for recruitment or training. Fluency in English, verbal and written; additional languages may be required for some centers. Use of independent judgment in clinical decision‑making. Pay Range Salary: $111,140 – $158,771 annually. Final compensation will depend on experience, education, geographic location, and other factors. The position may also be eligible for bonuses or commissions.
