Hospice RN Case ManagerSt. Mary's Home Care & Hospice is seeking a full time Hospice RN Case Manager. $10K Sign on bonus offered. Signing bonus is not applicable to internal employees, former employees who have resigned in the last year or applicants sourced from outside firms.Hospice RN Case Manager position summary: Responsible for case managing a team of hospice patients. RN case manager will collaborate with interdisciplinary team and attend weekly Interdisciplinary meetings. RN Case manager will interact with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms. Position is full time primarily Monday-Friday.
