Responsibilities
- Conduct initial and ongoing occupational therapy evaluations for residents in the skilled nursing setting.
- Develop, implement, and adjust individualized treatment plans to address functional impairments and rehabilitation goals.
- Deliver evidence-based interventions focused on activities of daily living, mobility, balance, endurance, and safety with transfers.
- Provide patient and family education on exercises, self-management strategies, safety, and use of adaptive equipment.
- Maintain timely, accurate documentation of evaluations, progress notes, goals, and discharge planning in the electronic medical record.
- Collaborate with an interdisciplinary team (nursing, physical therapy, social work, and case management) to coordinate care and facilitate smooth transitions.

Requirements
- Active Occupational Therapist license in New Jersey
- Degree in Occupational Therapy from an accredited program
- Experience in acute care or skilled nursing settings preferred
- Ability to work the listed schedule (08:00–16:30) and maintain guaranteed hours
- Strong communication, documentation, and teamwork skills
