Registered Nurse (RN) | Case Manager Salary : $85K & $5k Sign-On Bonus Productivity Bonuses: $100 per additional regular visit, $150 per SOC Schedule : Monday - Friday 8a - 5p Location : Redding, CA Must have Home Health or Hospice Experience What we offer our Registered Nurse (RN) | Case Manager Annual Salary of $85k + Productivity Bonuses (1 year employment required for entire initial sign-on) Generous PTO, Holiday Pay, Medical/Dental/Vision & 401(k) Benefits 1:1 nurse-to-patient ratios where you impact outcomes Create your own schedule Responsibilities Provide in-home direct patient care according to the provider’s prescribed plan of care (and nursing scope of practice) while closely monitoring progress/recovery. Assess and document patient conditions and report changes as needed (vitals, blood/stool/urine, glucose, etc.) Administer medication, insulin, and IV/fluids, documenting thoroughly Inspect and care for wounds, changing dressings and assisting with personal hygiene Assess patient for further injuries, infections, bedsores, etc. and deliver appropriate treatments Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes Participate in ongoing interdisciplinary assessment of the patient Participates in Quality Assessment and Performance Improvement (QAPI) program Supervises paraprofessional and licensed professional team members Qualifications Graduate of an accredited school of nursing and has a current license as a Registered Nurse in CA Two (2) years of current nursing experience preferred One (1) year management experience preferred Must have Home Health or Hospice experience Community Health and a wide range of Medical Surgical experience preferred, not required Ability to pass a Criminal Background Check, Physical, TB & Drug Screen Must have a current Driver's License and reliable transportation We're an equal opportunity employer.
