A Home Health Registered Nurse is needed for a full-time, 13-week contract providing quality care to patients in a field-based setting. This role is central to promoting patient wellness and safety in their homes, focusing on comprehensive evaluation, care coordination, and patient education. Bring your clinical expertise, initiative, and compassion to support recovery and improve outcomes for diverse patient populations.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based: Daily travel to patient homes within your assigned territory
• Caseload: 2532 points weekly (typically 35 patient visits per day)
• Electronic Documentation: Home Care Home Base (HCHB)
• Assessment Tool: OASIS

Responsibilities:

• Complete OASIS assessments and develop personalized care plans
• Deliver skilled nursing care (wound care, medication administration, IV therapy, chronic disease management)
• Monitor patient status, identify changes, and communicate effectively with interdisciplinary care teams
• Provide education to patients and families on conditions, treatments, and self-care
• Maintain accurate, timely documentation in HCHB
• Participate in case conferences, meetings, and ongoing training
• Ensure compliance with all agency and regulatory standards
• Collaborate with therapists, social workers, and aides for coordinated care

Required Qualifications:

• Active RN license in good standing
• At least 1 year of recent Home Health nursing experience (field experience preferred)
• Proficient in HCHB and OASIS documentation
• Reliable transportation, valid drivers license, and auto insurance
• Excellent assessments, communication, and organizational skills
• Ability to manage a flexible field-based schedule

Preferred:

• BSN degree
• Experience with managing a caseload of 2532 points weekly
• Familiarity with Medicare and home health compliance

$1,954 - $2,067/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
