Focus: Chronic care management (CCM) and longitudinal primary care Licensure required: Active Illinois APRN and RN, board certification (FNP-BC via ANCC or equivalent), active DEA Systems: EPIC (HPC's EHR) Comp range referenced: $125K–$130K Nice-to-haves called out in the interview prep: value-based care/quality metrics familiarity (ACO, MSSP, HEDIS/Star ratings, payer programs like APP, Stellar, UHC-Optum, Meridian); RPM/CCM program experience; comfort transitioning from acute/episodic care to longitudinal panel management #J-18808-Ljbffr
