Completion of an accredited residency program (Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics)

Full-time Primary Care Physician role with a Monday–Friday schedule providing comprehensive, value-based care to senior patients in a community-focused setting.
\n Job Overview
\n As a Primary Care Physician, you will provide holistic, relationship-driven care to a smaller panel of senior patients, allowing you to make a meaningful impact on their health and quality of life. You will conduct comprehensive assessments, develop personalized care plans, and collaborate closely with an interdisciplinary team to ensure coordinated, patient-centered care. This role focuses on preventive and long-term care rather than high patient volume, with an average of eight patients per day and no hospital rounds. You will also support care transitions and participate in a rotating on-call schedule by phone. This is an ideal opportunity for a physician who values deeper patient relationships, team collaboration, and a mission-driven approach to care.
