Initiates and facilitates appropriate drug dosing and manages medication‑related pharmacist consults. Leads and facilitates pharmacy practice programs, direct patient care services, and medication systems. Completes critical medication order scrutiny, drug profile review, and patient monitoring to identify, prevent, or mitigate medication‑related problems. Verifies and enters medication orders, proactively communicating with medical staff to resolve problems. Provides direct patient care, including pharmacotherapy assessment, therapeutic monitoring, pharmacokinetic dosing, transitions‑of‑care. Precepts, mentors, and educates pharmacy students, residents, and staff, and provides continuing education to interdisciplinary team members. Contributes to scholarly activity and professional advancement through publications, presentations, and committee involvement. Identifies, mitigates, and reports potential and actual adverse drug events, adverse drug reactions, and medication errors. Accurately manages medication inventories and prepares and dispenses patient‑specific medication doses. Leads and facilitates pharmacy practice programs, directs patient care services and medication systems. Other duties as assigned.
