MJHS seeks an Admission Nurse to initiate and resume high-quality home health services for patients referred to the agency in Queens, NY and occasionally Brooklyn. You will create the initial, interdisciplinary plan of care in collaboration with physicians or approved practitioners, ensure regulatory compliance, and provide expert community health nursing care and teaching to patients in their homes. #J-18808-Ljbffr MJHS
