Job Description

The unit has a capacity of 40 beds and typically requires collaboration between case managers and social workers.

Responsibilities

• Coordinate patient care and services within the healthcare team.

• Collaborate with medical professionals, patients, and families to optimize care delivery.

Required Experience

• Minimum of 1 year of acute care case management experience.

Certifications / Licensure
