Nurse Navigator, Metabolic and Weight Program

We learn, grow, and achieve more – in our careers and in our communities.Job Description Summary:Monday - Friday, No Weekends & No HolidaysSchedule: 8:00am - 5:00pmWe are seeking a compassionate and motivated Nurse Navigator to join our Medical Weight Management team at McConnell Heart Health Center. In this dynamic role, you will serve as a key point of contact for patients navigating care related to metabolic health, including weight management, hypertension, and hyperlipidemia. Working in a collaborative, interdisciplinary environment, you will help guide patients through their care journey while contributing to a growing, innovative area of preventative healthcare.In this predominantly outpatient setting, you will manage patient referrals, review medical records to determine appropriate care pathways, and connect patients with the right specialists within our program. You will spend time communicating with patients to explain services, gather clinical information, and coordinate scheduling, while also supporting pre-visit preparation and providing in-clinic education. This role offers a unique opportunity to blend clinical expertise with patient engagement, allowing you to make a meaningful impact on patient outcomes while helping shape evolving processes within the program.The ideal candidate is an RN who thrives in a self-directed environment, values patient interaction, and is passionate about metabolic health and preventative care. Experience in an ambulatory or specialty practice is preferred, and cardiac knowledge is a plus! Responsibilities include management/navigation of patients, education and advocacy, and coordination of care with managing physicians, assurance of high-quality care, continuous process improvement, optimal productivity, efficient utilization of resources and customer satisfaction in the Metabolic programs for OhioHealth. Coordination of care will be over an extended period of time and involves multiple modalities of treatment screening referrals, patient communication, direct patient rooming functions, in person patient education, physician communication, following up on order placement and test completion, and a component of data tracking and registry work.Responsibilities And Duties:70%Advocacy, Education & Support: Provides a customer focused and supportive environment for weight loss activities. Assesses learning needs, provides education, assesses learning achieved, and provides support and navigation to surgical weight management program patients and families. Coordination of care with managing physicians and multi-disciplinary team. Establishes system for coordinating the patients care throughout continuum, spanning each level of care in which care is provided and coordinating interdisciplinary communication Expands patient education throughout continuum and assists with documentation in patient record. Initiates and contributes to modifications/changes in caregiver practice patterns defined by outcomes: improved continuity and quality of patient care; increased participation and satisfaction of patients and families; efficient allocation of resources, appropriate length of stay and improved communication between disciplines. Routinely and systematically measure function and well being of patients and disease population-specific clinical and financial outcomes, at appropriate time intervals. Addresses, plans and implements clinical process improvement initiatives through Multi-Disciplinary Teams.30%Process Improvement & Leadership: Participates in clinical quality and process improvement activities Identifies educational and marketing needs with multidisciplinary team throughout continuum of care. Collaborates with leadership triad Physician Medical Director, Nurse Manage r to assure successful implementation of projects. Acts as leader, mentor to staff and other colleagues in connection with initiatives associated with related patient population. Remains current on new/evolving technology, healthcare issues and standards. Plans and organizes work effectively. Meets expectations for internal and external outcomes. Fosters performance/growth, promotes participation management. Develops/mentors others. Bachelor of science in Nursing from a four-year college or university. Licensed to practice as Registered Nurse in Ohio; current Basic Life (BLS) provider Clinical knowledge of surgery processes and weight management issues.
