Develop and oversee individualized social service care plans, supporting residents' emotional and social well-being. Act as an advocate for residents, ensuring their needs and concerns are addressed and respected. Coordinate with healthcare providers, family members, and community resources to support comprehensive care. Ensure compliance with state and federal regulations and maintain accurate documentation. Lead and mentor the social services team, promoting a collaborative and compassionate care environment. What We're Looking For? Education : Bachelor's degree in Social Work or a related field. Experience : Prior experience as a Social Services Director or similar role in a healthcare or post-acute setting. Licensure : Licensed Social Worker in Alaska (or eligibility to obtain licensure). Compassionate Leader : Demonstrated commitment to resident-centered care and strong interpersonal skills. Organized and Detail-Oriented : Excellent organizational skills and the ability to manage documentation and compliance requirements effectively. Team Player : Ability to work effectively with multidisciplinary teams and establish positive relationships with residents and families.
