PT - Physical Therapist

The Outpatient Physical Therapist is responsible for evaluating patients, developing, implementing, and modifying treatment plans in accordance with patient needs, goals, and physician directives. The role includes delegating and directing discipline‑specific therapy services to design functional treatment programs ensuring effective community reintegration. The therapist also educates patients, families, students, and coworkers while performing professional duties associated with patient care. Responsibilities Comprehensive evaluation of assigned patients. Utilization of clinical reasoning to design appropriate treatment plans and goals. Clear, concise, and timely medical record documentation. Participation in student programs, providing a positive learning environment. Creation of individualized professional development plans. Adherence to discipline‑specific State Practice Acts and regulatory requirements. Active engagement in organizational performance improvement initiatives. Required Experience / Certifications / Licensure Degree in Physical Therapy from an accredited school (required). 1+ year of outpatient clinical experience (required). CPR Certification (required).
