Responsibilities:

• Design and implement individualized rehabilitation plans to meet the unique needs of each patient

• Conduct thorough evaluations to identify patient needs and establish goals

• Provide therapy services, including but not limited to occupational, physical, and speech therapy

• Collaborate with multidisciplinary teams to ensure comprehensive care

• Develop and maintain patient records, including progress notes and treatment plans

• Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions

Requirements:

• Experience as a Rehabilitation or Physical Therapist or related field

• Strong communication and interpersonal skills

• Ability to work in a fast-paced environment and prioritize tasks effectively

• $58 - $60 HOURLY (commensurate with experience)
