description : Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives. Serves as co-leader in interdisciplinary approach to patient groups. requirement : Day Shift, 5 x 8s, No weekends. M - F. Must know HCHB and OASIS. Must have 1 year of home health experience. No call. Mileage paid, per government rate.Education and Work Experience:Doctor of Physical Therapy (DPT) degree: Required; only if obtained license on or after January 2016; or foreign equivalent.Licenses/Certifications:Physical Therapy licensure in all applicable states of practice: RequiredCardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification from approved vendor per AH policy: RequiredValid Driver?s License (DL) and must be at least 21 years of age or older: RequiredFacility Specific License/Certifications:AHA-BLS or AHA-CPR: RequiredPhysical Therapy licensure in state of practice: Required
