Continuity Care Home Nurses, Inc., an RN-owned private duty home care agency serving Southern California since 1991, is seeking experienced Registered Nurses to join our clinical team on a per diem basis. We are looking for highly skilled, organized, and dependable nurses who enjoy managing patient care from admission through discharge while providing exceptional clinical services in the home setting. Responsibilities Conduct new patient assessments and admissions Complete comprehensive nursing evaluations Perform dressing changes, blood draws, and skilled nursing visits Manage Workers' Compensation cases and follow-up visits Coordinate patient care with physicians, insurance companies, case managers, and family members Supervise LVNs, CNAs, and CHHAs as appropriate Monitor lab results and communicate changes in patient condition Complete recertification and follow-up visits Provide clinical reports to the Director of Patient Care Services Maintain accurate and timely clinical documentation Additional Clinical Opportunities IV therapy and infusions Central line care Peripheral IV starts and restarts Heparin locks High-tech nursing cases Catastrophic injury and complex care management Qualifications Current California RN License Hospital experience required Home care/private duty experience preferred Strong clinical assessment skills Excellent documentation and time management skills Ability to work independently Strong organizational and multitasking abilities Reliable transportation and willingness to travel throughout Southern California Requirements Complete and submit paperwork within 24 hours of patient visits Conduct thorough and accurate patient assessments Follow assigned cases on an ongoing basis Maintain communication with physicians, insurance companies, and office staff Demonstrate professionalism, efficiency, and reliability Service Area Los Angeles County Orange County Ventura County Antelope Valley/Lancaster Area Surrounding Southern California communities What Makes Us Different No OASIS paperwork Flexible per diem scheduling Opportunity to manage and follow your own cases Supportive clinical leadership Diverse and challenging patient population Benefits 401(k) matching Dental insurance Health insurance Paid time off Vision insurance Bilingual candidates are encouraged to apply but are not required.
