Assessment RN / Case Manager

We are seeking an RN Case Manager/Monthly Assessment Nurse to join our team! You will be responsible for the assessment, care plans, and documentation of assigned residents.

Position: Assessment RN / Case Manager

Location: Sonoma County

Hybrid Position, remote and on-site monthly visits

Conduct initial and monthly assessments on residents in assisted living facilities/Skilled Nursing facilities

Pay rate: $75/per assessment (20 mins)

This can be a part-time or full-time position, you may accept up to 60 residents per your caseload, work on your own time.

Responsibilities:

Visit residents at a local assisted living facility, monthly
Monthly assessment, semi-annual comprehensive assessment with a care plan
Monitor and report changes in patient symptoms or behavior.
Communicate with collaborating physicians, administrators, Home Health agency, Hospice agency, POA, family members regarding patient care.
Educate patients about health maintenance and disease prevention
Facilitate referrals to other healthcare professionals and medical facilities
Maintain accurate patient medical records
Provide advice and emotional support to residents and their family members
You are in charge of your schedule, flexibility is welcome.
