Role Description The Family Nurse Practitioner at Verve Health will provide patient-centered primary care services, including comprehensive health assessments, diagnosis, and management of acute and chronic conditions. Day-to-day responsibilities include conducting in-depth patient evaluations, ordering and interpreting diagnostic tests, developing individualized treatment plans grounded in functional medicine principles, and collaborating with physicians and other clinicians to coordinate care. The practitioner will educate patients on lifestyle interventions, prevention strategies, and long-term wellness plans, while documenting care in the electronic health record and participating in quality improvement initiatives. Strong foundation in Primary Care, with experience managing acute and chronic conditions across the lifespan. Knowledge of Medicine and functional/regenerative health concepts, with the ability to integrate evidence-based and holistic approaches. Experience in various healthcare settings, working with teammates to drive better patient outcomes. High relationship-based care with sound decision‑making skills. Current, unrestricted Nurse Practitioner license in Indiana; national certification as an FNP or equivalent; BC‑FNP is a plus, but not required. Advanced degree in Nursing or related field from an accredited program. Strong communication, interpersonal, and patient education skills, with a commitment to culturally sensitive and inclusive care. Ability to work on-site, video, or telephone, demonstrating professionalism, reliability, and a team‑oriented mindset. Experience or interest in functional medicine, preventive care, employer health, and cost‑effective care models is highly beneficial. Bonus - Existing Patient Panel: Nurse practitioners who can bring an existing patient panel to Verve Health will be given priority consideration.
