Physical Therapist - Travel Contract

Role Overview:

A traveling Physical Therapist providing home health services in Lancaster, OH.

Responsibilities:

Conduct Start of Care visits, evaluations, recertifications, and treatment sessions in patients' homes. Manage documentation using HCHB platform and ensure quality patient care across multiple counties.

Qualifications:

Must hold an accredited Physical Therapy degree and an active Ohio license. Minimum of one year home health experience with HCHB knowledge required. BLS certification may be necessary.

Skills & Requirements:

Ability to perform clinical assessments, navigate documentation platforms, and travel reliably with mileage reimbursement. Preferred experience in lymphedema therapy.

Key Duties:

Provide patient evaluations, treatments, and recertifications in home settings within designated coverage areas. Maintain professional attire (scrubs) and adhere to schedule requirements.
