What You'll Be Doing (High Level)
- Providing skilled PT care in patients' homes
- Completing admissions and routine visits
- Developing and carrying out individualized care plans
- Collaborating with nurses, therapists, and providers Participating in interdisciplinary team meetings (The formal job description comes laterthis is about the experience.)

What You'll Need
- Oregon PT license (or ability to obtain before start date)
- 12 years PT experience (home health preferred, not required)
- Current CPR certification
- New PT graduates encouraged to apply
