Home Infusion RN Liaison: Hospital-to-Home Advocate

Baptist Health Floyd is seeking a Home Infusion RN Hospital Liaison to join our Home Infusion team. The role bridges hospitals and home infusion services, advocates for patients, coordinates education, and ensures seamless transitions from hospital to home. Shifts are Monday through Friday, 9:00 am to 5:30 pm with on-call weekends for new referrals leaving the hospital. The liaison collaborates with physicians, pharmacists, and case managers to deliver individualized care and maintain regulatory #J-18808-Ljbffr Baptist Healthcare System
