As our hospice census continues to grow in the Salt Lake City area , we are looking for a compassionate and dependable Registered Nurse Case Manager to join our team.

What You’ll Do:

As a Hospice RN Case Manager, you will coordinate and provide individualized nursing care for a caseload of hospice patients in their homes and residential care settings throughout the Salt Lake City area.

• Perform comprehensive nursing assessments and ongoing evaluations of hospice patients.
• Develop, implement, and update individualized plans of care in collaboration with patients, families, physicians, and the interdisciplinary team.
• Provide skilled nursing care with a strong focus on comfort and symptom management.
• Recognize changes in patient condition and respond appropriately and promptly.
• Educate and support patients and families regarding medications, symptom management, disease progression, safety, and what to expect throughout the hospice journey.
• Coordinate care with LPNs, CNAs, social workers, chaplains, physicians, facilities, pharmacies, DME providers, and other members of the hospice team.
• Communicate changes in condition and patient needs to the appropriate members of the interdisciplinary team.
• Participate in interdisciplinary group (IDG) meetings and collaborate in developing and updating the plan of care.
• Complete accurate and timely clinical documentation.
• Help ensure patients and families have the medications, equipment, supplies, education, and support they need.
• Provide guidance and supervision to LPNs and hospice aides as appropriate.
• Participate in the on-call rotation as scheduled.
• Maintain compliance with hospice regulations, agency policies, and professional nursing standards.

What We’re Looking For:

• Current Registered Nurse license in the State of Utah .
• Graduate of an accredited nursing program.
• Nursing experience preferred; hospice, home health, palliative care, oncology, geriatrics, or other relevant experience is a plus.
• Strong clinical assessment and critical-thinking skills.
• Compassionate and comfortable caring for patients and supporting families at end of life.
• Excellent communication and interpersonal skills.
• Strong organization, time-management, and prioritization skills.
• Ability to manage a caseload while recognizing and responding to changing patient needs.
• Ability to work independently in the field while also being an active member of a collaborative team.
• Dependable, flexible, and committed to following through for patients and families.
• Current driver’s license, reliable transportation, and automobile insurance.
