Job Summary

Homeside Rehab is seeking a compassionate and dedicated Physical Therapist Assistant (PTA) or Occupational Therapy Assistant (OTA) to provide home-based therapy services in Pound Ridge, NY 10576 .

Job Responsibilities

• Provide skilled therapy services under the supervision and direction of a licensed Physical Therapist or Occupational Therapist

• Carry out established plans of care and treatment interventions based on patient needs and goals

• Assist patients with improving mobility, strength, functional independence, activities of daily living, and overall quality of life

• Monitor patient progress and communicate changes or concerns to the supervising therapist

• Maintain accurate and timely documentation of all patient care in accordance with regulatory and company requirements

• Collaborate with supervising therapists and the clinical team to ensure coordinated and effective treatment

• Communicate professionally with office staff, patients, and caregivers regarding scheduling and patient care

Job Qualifications

• Active New York State PTA or OTA license/certification — required

• Basic computer proficiency for electronic documentation

• Strong interpersonal and communication skills

• Ability to work independently in a home-based setting while following the established plan of care

• Reliable transportation for home visits

• Home care experience preferred

• Minimum 1 year of experience in Home Health, Outpatient, or Skilled Nursing Facility (SNF) settings preferred

• Job Type: Full-time, Part-time, Per Diem

• Pay: $65–$80 per hour

• Flexible Scheduling: Choose your own hours

• Monday through Friday
