EPIC charting system experience is required. Description: The Home Hospice Case Manager is responsible for providing quality care to patients in a home setting. Experience in home hospice case management is essential for this position. Candidates must have a valid RN license and BLS certification. This role entails managing a census of 35-45 patients. Familiarity with the EPIC charting system is required. Candidates should bring a supportive and compassionate approach to patient care. This position may require flexibility and the ability to float based on facility needs. An emphasis is placed on teamwork and interdisciplinary support in patient care.
