Nurse Practitioner *5K Sign on Bonus*

Job Summary An Advanced Practice Provider is a licensed and board‑certified nurse practitioner or physician’s assistant specifically assigned to Longevity Health Plan members to execute the Institutional Special Needs Plan Model of Care. The APP medically manages a panel of Longevity Health Plan enrolled nursing home level of care members in partnership with the primary care team and delivers the Longevity Health Plan Model of Care. The APP conducts comprehensive assessments, develops individualized plans of care, attends nursing facility care team meetings, communicates regularly with residents’ families, and ensures the care team understands the member’s goals of care and needs. The APP focuses on treating in place, uses clinical practice guidelines to guide treatment decisions, and documents members’ health status, functional status, and diagnoses. As part of the clinical team the APP is supported by the APP Clinical Manager, Market Director of Clinical Services, and local medical director who provide education and guidance on best practices to effectively care for members. Key Tasks and Responsibilities Provides collaborative primary care and services to a panel of Longevity Health Plan members, treating in the most appropriate setting based on the member’s goals of care. Executes and delivers contracted plan Model of Care activities, develops and implements advanced care planning, and embraces a treating‑in‑place model. Facilitates and administers clinical operations functions—including prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, and promotion of membership growth. Serves as a trusted advisor and advocate for members, loved ones, and nursing home staff, educating care teams on member needs and early identification of changes in condition. Delivers effective and frequent communication and coordination with members’ PCPs, loved ones, and care team members regarding care needs, goals of care, and treatment plan changes. Supports overall market goals and balances member caseload demands with other team member support needs. Promotes Longevity Health with members, loved ones, nursing facilities, and other stakeholders. Functions autonomously and is accountable for practice. Establishes meaningful relationships and can function in the nursing home environment. Is member‑centric, customer‑service oriented, and ensures effective, high‑quality care for Longevity Health members. Performs other related duties as assigned. Supervisory Responsibilities There are no supervisory responsibilities for this position. Credentials & Coverage State‑unrestricted licensed Advanced Practice Nurse with National Board certification (AANP, ANCC, or AACN) as an Adult, Family, Geriatric, or Primary Care nurse practitioner required. An acute‑care nurse practitioner with nursing home/geriatric or hospice palliative experience is also acceptable. State‑unrestricted licensed Physician’s Assistant with NCCPA board certification and nursing home/geriatric or hospice/palliative experience required. Active DEA license registered in the state of practice is required. Must have an active national provider number (NPI) and an active Medicare provider number. Must be eligible to participate in the Medicare program as a provider. Valid state driver’s license with a good driving record and proof of automobile insurance required. Proof of coverage must be provided on an annual basis. Must be willing to drive up to one hour to and from work daily. Must be present in SNF communities/buildings during business hours daily. Education and Training Master of Science in Nursing as a Nurse Practitioner from a nationally accredited program OR Master’s Degree from a nationally accredited program. Knowledge and Experience Minimum of 6 months experience as a Nurse Practitioner or Physician’s Assistant with experience in geriatric/hospice or palliative care populations. Experience working in the nursing home environment is strongly desired. Computer/technology proficiency required. Ability to engage and communicate effectively with physicians, patients, and family members—excellent people skills. Self‑directed but functions well in a team environment. Excellent clinical skills—assessment, diagnosis, treatment, and prescribing for the chronically ill population.
