PRN Registered Nurse (RN) or Licensed Practical Nurse (LPN)

South Bend, Elkhart & Goshen, IN

Current Opportunity

We are seeking a PRN Registered Nurse (RN), preferably, or Licensed Practical Nurse (LPN) to provide intermittent skilled nursing visits in the Elkhart/Goshen area , with the ability to assist with visits in South Bend when needed.

An RN is preferred to assist with patient admissions; however, qualified LPN candidates are also encouraged to apply.

Current Scheduling Needs

The initial schedule would be approximately 2 days per week .

Nursing visits are compensated per visit rather than hourly , with the current schedule including:

• One nursing visit on Fridays

• One nursing visit every other Sunday

• Additional PRN visits may become available as patient needs grow

Pay: $40–$50 per nursing visit through Firefly Home Health

There may also be opportunities for care management work through First Call Care Management , which pays approximately $45 per hour .

Nursing Responsibilities

• Provide intermittent skilled nursing visits in patients’ homes

• Complete nursing assessments and monitor changes in patient condition

• Follow individualized plans of care and physician orders

• Provide education and support to patients and their families

• Maintain accurate and timely clinical documentation

• Communicate with physicians, care coordinators, and other members of the care team

• Assist with admissions and start-of-care visits when qualified and needed

• Travel throughout the Elkhart/Goshen area and occasionally to South Bend for scheduled visits

Qualifications

• Active Indiana RN or LPN license in good standing

• RN preferred, particularly for admission visits

• Previous home health, skilled nursing, or geriatric care experience preferred

• Strong assessment, communication, and documentation skills

• Dependable, compassionate, and professional

• Reliable transportation and a valid driver’s license

• Ability to pass all required background screenings
