On Call Caregiver - With Vehicle

On Call Caregiver

Looking for an on-call caregiver to service the north side Chicago. Must live in close proximity to the office (office zip code 60640). Caregiver needs to have experience:

Personal care duties
Transfers using hoyer lift / gait belts
Companionship
Dementia experience (not mandatory but a plus)
Parkinson's experience (not mandatory but a plus)
Reliable transportation
