JOB SUMMARY The registered nurse in home health utilizes the nursing process—assessing, planning, implementing, and evaluating—to deliver care to patients in their residences. This role involves providing tailored care for individuals across all developmental stages, including adults aged 18-72 and seniors aged 72 and above, in accordance with established policies, procedures, guidelines, and nursing standards. The nurse is responsible for managing the care and case management of patients throughout their life stages, guided by their experience and competency evaluations. PRIMARY JOB DUTIES Conducts assessments, interprets data, plans, implements, and evaluates care based on the patient's age and diagnosis. Efficiently manages a caseload of patients while coordinating care with a multidisciplinary team. Communicates information appropriately, adhering to agency policies and discipline-specific guidelines. Contributes to the effectiveness of programs. Organizes and executes work tasks effectively and efficiently. Adjusts schedules to optimize agency performance. Exhibits a daily commitment to the agency's core values. Maintains positive interpersonal relationships with all agency members. Promotes and maintains customer satisfaction. Consistently demonstrates the agency's mission, vision, and values in daily activities. JOB SPECIFICATIONS Education: Must be a graduate from an accredited nursing program, whether an Associate Degree, Diploma, or Bachelor of Science in Nursing. Licensure / Certifications: A current license to practice professional nursing in the state of care provision (NC/SC) is required, along with CPR certification. Experience: A minimum of one year as a registered nurse and at least two years of clinical experience is necessary. Additional experience may include roles such as LPN, CNA, military medic, EMT, or similar. Home health experience is preferred. For those with less than one year of RN experience, one year of clinical experience as an LPN is required (internal use only). Therapy Assistants (PTA, OTA) with one year of home health experience and at least six months of RN experience may also be considered (internal use only). Essential Technical / Motor Skills: Requires hand-eye coordination for administering injections, using a computer, etc. Must be proficient in English communication and able to operate patient care equipment, including safely transferring and assisting patients. Interpersonal Skills: Ability to foster positive interactions with patients, their families, physicians, and staff to ensure effective patient care. Essential Physical Requirements: Must be capable of transferring or maneuvering objects weighing at least 50 pounds as part of patient care. Frequent lifting, moving, and positioning of patients is required, along with physical effort for bathing and ambulating patients. Occasional overhead reaching, stair climbing, and fine motor manipulation may be necessary. Essential Mental Abilities: Ability to assess patient conditions, develop care plans, select appropriate interventions, evaluate responses to treatment, and educate patients about their conditions and recovery. Requires strong cognitive skills, including short- and long-term memory, prioritization, and the ability to learn new patient care techniques. Essential Sensory Requirements: Must be able to visually assess patients and utilize sight for implementing and evaluating care plans (e.g., changing dressings, starting IVs). Hearing is essential for auscultating lung and bowel sounds, responding to alarms, and communicating effectively with patients, families, and staff. Exposure to Hazards: Potential exposure to noise, bloodborne pathogens, body fluids, infectious diseases, and needle punctures. May encounter dangerous animals and traffic hazards during home visits, as well as situations that could pose personal safety risks. Weather extremes and temperature changes may also be experienced. Hours of Work: Variable schedule from Monday to Friday, with weekends and holidays as needed. Flexibility is required to meet staffing demands. Population Served: Care is provided to adolescents, adults, geriatrics, and pediatrics. Additional Requirement: A valid driver's license from North Carolina and access to a reliable vehicle are mandatory.
