Position Title: RN

Location: Greater Heights, TX 77008

Duration: 13 weeks

Work Schedule: Days

Guaranteed Hours: 40 hrs.

Pay rate: $61 - $68/hr.

Responsibilities for this position include, but are not limited to:

• Manage all discharge planning needs for patients in an inpatient rehabilitation setting.
• Complete initial discharge planning assessments within two days of admission.
• Participate in the initial interdisciplinary team meeting by day four of the patient's stay and attend weekly meetings thereafter.
• Coordinate home health services, outpatient therapy, durable medical equipment (DME), and dialysis chair setup and confirmation.
• Complete post-discharge follow-ups at 72 hours and 30 days.
• Conduct continued-stay insurance reviews and authorizations using payer portals.
• Reissue the Important Message from Medicare as required.
• Coordinate care with physicians, nursing staff, rehabilitation therapists, and other interdisciplinary team members.
• Support timely discharge planning and manage quick patient turnaround times. The average patient length of stay is approximately 11.2 days.
• Utilize WellSky for documentation and case management activities.
• Support weekend and after-hours admissions as needed.

Requirements:

• Active Texas RN license.
• Current BLS certification.
• Minimum 2 years of experience in acute care case management, inpatient rehabilitation case management, and/or utilization review.
• Experience with discharge planning, care coordination, and continued-stay reviews.
• Familiarity with insurance payer portals, including Availity and Home & Community.
• Experience using WellSky is preferred/required per client confirmation.
• Strong communication, organization, documentation, and interdisciplinary coordination skills.
• Ability to work Monday through Friday, 8:00 AM–5:00 PM.
• Flexibility to work between the Houston and Dallas sites.
