Pediatric Complex Care Nurse Patient Navigator

Overview The Nurse Patient Navigator serves as an advocate, interpreter, educator, and counselor for a group of patients, acting as their single point of contact and support system throughout treatment. Key Responsibilities Advocate, interpret, educate, and counsel assigned patients and caregivers. Manage the care of assigned patients in collaboration with a multidisciplinary team to ensure high quality and comprehensive healthcare and ancillary services. Serve as the single point of contact for assigned patients and caregivers. Remain a support system to patients and caregivers throughout the patient’s treatment. Use clinical and nursing knowledge and skills in performance of job responsibilities. Follow documented physician/licensed independent provider medical/treatment orders. Practice within the boundaries of nursing regulations in Virginia and guided by the ANA Code of Ethics, national nursing practice standards, and VCUHS policies and procedures. Organize and plan work with input from the patient/family, identifying specific outcomes. Demonstrate sound clinical judgment based on nursing knowledge and experience. Qualifications Current RN licensure in Virginia (or eligible/compact state). AHA BLS certification. Specialty nursing certification in nurse navigation or an area of specialty within one year of hire. Minimum of three (3) years of relevant clinical experience in nursing, patient education, case management and/or navigation. Previous experience in a specialty area preferred. Previous nurse navigation or leadership experience preferred. Baccalaureate degree in Nursing from an accredited School of Nursing. Master’s degree in Nursing from an accredited School of Nursing preferred. Independent Actions and Practice All practice is guided by and follows the VCUHS policies and established practice guidelines. Physical Requirements Lift less than 20 lbs. Prolonged standing and seated activities. Frequent bending, walking (distance), climbing (steps, ladder, other). Reaching (overhead, extensive, repetitive). Strong recall, reasoning, problem solving, hearing, clear speech, legible writing, reading, logical thinking. Handle fast and steady pace environments, multiple priorities, frequent and intense customer interactions, noisy settings, and frequent change. Additional Requirements Positions are primarily weekday work but may require evening or weekend hours and travel to all VCUHS locations.
