RN Case Manager: Discharge & Care Coordination

AdventHealth in Overland Park, KS seeks an RN Case Manager to coordinate discharge planning and ensure smooth transitions across inpatient to post-acute care. You will lead rounds with the care team and educate patients and families about options and resources. Requirements include an ASN and preferably a BSN; 2+ hospital nursing years with CCM/ACM preferred.
