Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients using interviews, observations, and reviews of medical records to understand emotional, social, and environmental factors affecting recovery.

• Collaborate closely with an interdisciplinary rehabilitation team to design and implement customized treatment plans that account for each patient's psychosocial and rehabilitation needs.

• Facilitate access to essential community resources, including vocational rehabilitation services, support groups, and housing assistance, enhancing the patients' pathways to recovery.

• Offer guidance and education to patients and their families regarding post-rehabilitation services and available community resources.

• Actively participate in discharge planning, ensuring smooth transitions for patients returning home or moving to alternative care settings.

Compensation and Benefits\n
