Registered Nurse - Home Health

Job Description

Job Summary:
Provide comprehensive home health nursing care to patients in their residences, ensuring delivery of quality services aligned with clinical standards and protocols. This role involves managing high patient volumes with a focus on compliance and personalized patient care.

Responsibilities:

• Assess and monitor patients' health status in their home environment.
• Administer medications and treatments following physicians' orders and regulatory guidelines.
• Develop, implement, and evaluate individualized care plans.
• Document patient interactions, interventions, and progress accurately in electronic health records.
• Collaborate with interdisciplinary teams to coordinate care and ensure patient safety.
• Adhere to all compliance policies, including mandatory certifications and local licensing requirements.

Qualifications:

• A valid active CA RN license in hand at the time of submission.
• Minimum of 2 years of home health experience with high-volume case loads, including at least 1 year of travel home health experience.
• Recent OASIS experience, including knowledge of G-Section, GG Section, and M18, with the ability to pass the OASIS exam.
• Current BLS certification.
• Strong clinical assessment, documentation, and communication skills.
