Clinical Pharmacist M-F 10:00 a.m. – 6:30 p.m. Job Description Summary The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of Option Care, pharmacy practice professional standards, and applicable regulatory entities. This entry‑level position is in the alternate setting and coordinates patient services with the Nursing Department, monitors patient progress toward goals, and educates patients, caregivers, staff, and other professionals regarding appropriate medication use. Job Responsibilities Determine the suitability of individual patients for home care, collecting and organizing all patient‑specific information needed to make the determination. Design, recommend, monitor and evaluate patient‑specific pharmacy care plans for pharmacotherapy of home care patients; recommend modifications in prescribed medication therapy or monitoring parameters when appropriate; assist in preventing, detecting, and resolving medication‑related problems. Modify pharmacy care plans for home care patients based on evaluation of monitoring data and other patient‑specific information; competent in pharmacokinetic principles, formulas, and lab values. Provide medication‑use education to Option Care staff, other professionals, home care patients and/or their caregivers using effective patient education techniques. Ensure continuity of pharmaceutical care to and from the home and other patient‑care settings, including preparation and dispensing of medications, calibrating equipment, preparing labels, inspecting final medication, and compliance with state/federal laws and USP standards. Manage and troubleshoot medication administration equipment and medication‑related equipment, program and maintain the equipment, advise others, and report incidents, errors, and discrepancies with perpetual inventory systems. Respond to requests for drug information from health care providers and home care patients with concise, timely answers based on literature analysis; attend educational seminars or CEU opportunities to promote professional competency and comply with state mandated CEU requirements. Manage investigational drug products in accordance with established regulations and Option Care policies; utilize pharmacy support personnel, prioritize workload, and ensure accuracy of their work. Comply with accreditation, legal, regulatory, and safety requirements in all activities. Maintain confidentiality of patient and proprietary information and observe legal and ethical guidelines for safeguarding data. Supervisory Responsibilities This position has no supervisory responsibilities. Basic Education & Experience Requirements Graduate of an accredited School of Pharmacy. Current, active Pharmacy license as granted by the State Board of Pharmacy. One of the following: (a) At least one year of sterile IV admixture procedure experience, (b) knowledge of non‑sterile compounding, immunizations, and medication therapy management (MTM), (c) completion of an ASHP accredited Pharmacy Residency Program, (d) completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy, or (e) equivalent experience as approved by the Vice President of Pharmacy and/or Sr. Vice President of Clinical Services. Basic Qualifications Experience providing customer service to internal and external customers, with meeting of quality standards and customer satisfaction evaluation. Basic level skill in Microsoft Word, Excel, and PowerPoint (operations such as opening, selecting, formatting, and printing documents). Travel Requirements Willing to travel up to 5 % of the time for business purposes within the state. Preferred Qualifications & Interests Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy. Experience in home care, hospital, or other clean‑room setting. Management Requirements At least one year of experience in indirect management of team members, including assistance in the development, training, and assignment of work to other team members. May perform other duties as assigned.
