Job Summary: The Pain Management Clinical Pharmacist will provide direct patient care and pharmaceutical services. The Clinical Pharmacist will maintain comprehensive medication therapy management and services under a defined scope of practice. In addition, will review electronic medical records to monitor drug therapy for appropriateness, and adverse drug reactions.
Required Qualifications:
- Education :
- Doctor of Pharmacy (Pharm.D.)
- Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE) or a Foreign Pharmacy Graduate Examination Committee (FPGEC) certificate and active licensure from a U.S. state, territory, or district.
- Licensure:
- Current, full, active, and unrestricted license to practice as a clinical pharmacist in one of the 50 US States and/or territories
- Certifications:
- Basic Life Support (BLS)
- Internship/Residency: Completion of a clinical pharmacy internship, residency and/or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy
- Requirements for additional Pharmacist privileges include one of the following:
- Certification by the Board of Pharmacy Specialties in one or more recognized practice areas appropriate for the privileges requested (e.g., Pharmacotherapy, Ambulatory Care Pharmacy, et.); or completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health-System Pharmacists or American College of Clinical Pharmacy; or completion of a Post-Graduate Master of Science degree with emphasis in medication
therapy management or direct patient care.
- Certification by the Board of Pharmacy Specialties in one or more recognized practice areas appropriate for the privileges requested (e.g., Pharmacotherapy, Ambulatory Care Pharmacy, et.); or completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health-System Pharmacists or American College of Clinical Pharmacy; or completion of a Post-Graduate Master of Science degree with emphasis in medication
U.S. Citizenship is required
Key Responsibilities:
- Assess patients response to drug therapy and planning drug therapy based on physician-established diagnoses.
- Order and evaluate laboratory tests necessary to evaluate drug therapy effects and outcomes.
- Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states (e.g., hypertension, hyperlipidemia, anticoagulant, diabetes, asthma, refill clinics, etc.), in cooperation with the medical staff.
- Assess metabolic needs and order therapeutic enteral or parenteral nutrition products in the inpatient setting.
- Evaluate medical and medication histories for drug-related problems and adjusting drug therapy accordingly.
- Consult with other healthcare providers (e.g., physicians, dietitians, nurses, physical therapists, etc.) about patient treatment needs or options.
- Perform a full range of pharmacist procedures.
- Review patient profiles at the time of dispensing in order to closely monitor medication therapy.
- Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients.
- Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.
- Provide medication and healthcare related in service training to clinical staff on new medications on the market; appropriate use of medications; comparison of current medication therapies; and other topics as deemed appropriate by the department head.
- Consult with other specialty practitioners to provide optimal patient pharmaceutical care.
Additional Duties :
- Conduct a clinical interview with patient, family, and/or caregiver(s) to identify information necessary to design a pharmacotherapeutic treatment plan.
- Order and evaluate appropriate laboratory tests to assess clinical response, development of adverse drug reactions, therapeutic drug levels, and complete all appropriate clinical reminders.
- Document and communicate all information as appropriate; pain patient agreements, pharmacotherapeutic treatment findings, and recommendations.
- Educate patients and their families/caretaker(s) on medications and non-drug therapy and provide appropriate written drug information: counsel on self-monitoring techniques and lifestyle changes to promote wellness; emphasize medication adherence and provide compliance aids, such as medication charts and pill boxes.
- Extend existing prescriptions (psychotropic and non-psychotropic medications) as appropriate and ensure the patient receives follow-up appointment(s) with the appropriate individual(s).
- Order consults and referrals as appropriate.
- Participate in an on-site designated Extension for Community Healthcare Outcomes (ECHO) telementoring training program.
Work Environment and Schedule: On site, Monday- Friday, 0630 -1800
Compensation & Benefits: Up to $161,00 per year, depending on experience and qualifications.
Physical requirements: Prolonged standing and walking; frequent computer and telephone use; manual dexterity for handling medications and pharmacy equipment; ability to read medication labels and patient records; occasional lifting/carrying up to 25 pounds; and ability to bend, reach, and move throughout the pharmacy/clinical setting while maintaining attention to detail in a fast-paced environment.
