Key Responsibilities

• Plan and conduct segments of physical therapy treatment programs designed to restore and improve physical, social, and mental functions
• Meet established department objectives while delivering high-quality patient care
• Serve as co-leader in an interdisciplinary approach to patient groups
• Document patient progress and treatment plans utilizing the EPIC electronic health records system
• Travel to patient locations as required for home health visits

Required Qualifications

• Doctor of Physical Therapy (DPT) degree required for those who obtained licensure on or after January 2016, or foreign equivalent
• Active Physical Therapy licensure in all applicable states of practice
• Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification from an approved vendor
• Valid Driver's License; must be at least 21 years of age or older
• Proficiency with EPIC electronic health records system

Preferred Experience / Skills

• Prior experience in a home health physical therapy setting
• Strong interpersonal and communication skills for working within interdisciplinary care teams
