Position Overview
We are seeking an experienced Registered Nurse (RN) - Case Manager with strong hospital-based case management experience. The ideal candidate will have a minimum of 5 years of hospital case management experience and proficiency in Cerner/PowerChart and discharge planning platforms.
Key Responsibilities
- Manage a caseload of approximately 25-28 patients .
- Perform comprehensive patient assessments and develop individualized discharge plans.
- Coordinate care across interdisciplinary teams to ensure appropriate resource utilization and timely discharges.
- Facilitate communication between physicians, patients, families, and post-acute care providers.
- Complete PASRR (ASCEND) entries and ensure compliance with state and federal regulations.
- Utilize Cerner/PowerChart and WellSky (formerly NaviHealth) for documentation and care coordination.
- Apply PAE knowledge and ensure accurate documentation.
- Float within scope of practice as needed to meet facility requirements.
Requirements
- Experience: Minimum 5 years of hospital case management experience (Required)
- EMR: Cerner/PowerChart experience (Required)
- Experience with WellSky (formerly NaviHealth) (Required)
- PASRR entry experience (ASCEND)
- Knowledge of PAE processes
- Active RN license (Pending license not accepted)
- First-time travelers will be reviewed
- Local candidates considered on a case-by-case basis

- No weekend requirement
- No on-call requirement
- Block scheduling not available
