Hope The Mission in Los Angeles seeks a Housing-Focused Case Manager (PT & FT) to guide participants from interim housing to permanent housing using Housing First and trauma-informed approaches. You will work under the Program Manager, conducting assessments, developing action plans, and coordinating move-in logistics. Responsibilities include ongoing participant engagement, HMIS documentation, and collaboration with housing partners to shorten shelter stays and build long-term stability. #J-18808-Ljbffr Hope The Mission
