Registered Nurse - Case ManagerAbility to assess, evaluate and drive discharge plan. Coordinate services to the patient's appropriate level of care. Ensure that treatment plan is initiated, evaluated and re-evaluated daily. Follow the progress and lack thereof daily. Monitor length of stay and identify barriers to discharge. Follow Medicare regulatory guidelines regarding patient choice, IMM notices and MOONS.Schedule: Monday-Friday. Weekend: None. Call: None. Floating: Within facility/scope of practice as needed.Minimum experience: 3 years - Unit does not accept first time travelers.
