Physical Therapist (Home Health)
This is a 13-week contract position.
AM shifts, Monday to Friday, 5 shifts per week, totaling 40 hours.
We are seeking a dedicated Physical Therapist to join our home health team. The therapist will provide services to patients in Burnet, Bertram, and Lampasas, with potential coverage within a 60-mile radius of the facility.
Conduct patient evaluations and develop individualized treatment plans.
Administer therapeutic interventions to enhance patient recovery and mobility.
Document progress and modify treatment plans as necessary.
Participate in weekly case conferences and coordinate with other healthcare professionals.
Ensure compliance with all OASIS guidelines.
Home Health and OASIS experience required.
Homecare Homebase documentation experience preferred; candidates without this experience may be considered at a reduced rate with training provided.
Must have an active Physical Therapist license.
