Ambulatory Care RN Leader, Team-Based Care

- p Providence Medical Foundation is hiring a Team Based Care (TBC) Manager RN in Victorville, CA. The role conducts shared visits with providers and independent RN visits using established workflows and protocols. You will collaborate with the care team to coordinate and execute the plan of care in a patient-centered model.
- p The position requires California RN licensure, nursing degree, and at least 2 years of professional nursing experience. A BSN is preferred and ambulatory care experience is a plus. This is a full-time, on-site role within Providence facilities.
