Schedule/Hours:

• Weekend coverage may be required
• Coverage primarily in San Jose, Santa Clara, and adjacent cities
• Occasional travel outside designated area to secure a full caseload

Responsibilities:

• Assess, plan, and implement physical therapy services across client homes
• Complete and document OASIS assessments, recertifications, evaluations, follow-ups, and discharges
• Collaborate closely with physicians, patients, and care teams for optimal patient outcomes
• Maintain productivity expectations and comprehensive records, including required point system
• Willingness to see Covid+ and Covid PUI patients as needed

Qualifications:

• Current California Physical Therapy License
• Basic Life Support (BLS) certification
• Valid driver's license, auto insurance, and registration
• Up-to-date vaccinations: TB (annual), MMR, Varicella, HepB, TDap (within 10 years), Flu (annual)
• Drug screening and physical exam clearance
• Flexibility to travel across designated territories and work weekends as needed
