Overview
Licensed Practical Nurse, Long Term Care, Malone, NY, US. This travel temporary assignment starts 08/03/2026 for 6 weeks, with a weekly pay range of $2,029–$2,201. Guaranteed hours are not guaranteed. Shift: evenings, 5x8. Setting: Long-Term Care (LTC - Nursing Home).

Responsibilities
- Provide licensed practical nursing care to residents under supervision of an RN, within a LTC setting.
- Administer medications and monitor residents for adverse effects, documenting care per policy.
- Perform CNA duties as needed; assist with activities of daily living and mobility to support patient comfort and safety.
- Monitor resident status, observe vital signs, report changes promptly to the supervising nurse, and participate in care planning.
- Maintain accurate documentation in the charting system and uphold infection control and safety protocols.
- Collaborate with the care team to ensure continuity of care and support unit operations and patient care needs.

Requirements
- Active New York Licensed Practical Nurse license on file at time of submission (or eligible for NY licensure).
- AHA BLS certification.
- LTC experience; minimum 1 year of LPN experience required.
- Ability to cover CNA duties as needed to support patient care.
- Graduate of an approved LPN program (diploma or degree).
- Willingness to undergo comprehensive background checks (NY state, nationwide, county court searches, FACIS-3, and sex offender checks) with timelines specified in the posting.
