Physical Therapist - Home Health

Pride Health is hiring a Physical Therapist to support our client's home health program in Batesville, AR (Coverage Areas: 50-mile radius surrounding Batesville, AR). This is a 13-week assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!

Location: Batesville, AR (Coverage Areas: 50-mile radius surrounding Batesville, AR)
Duration: 13-week Contract (Possibilities of extension)
Pay rate: $50-56/hr. On W2 (Pay Rate is based on experience)
Schedule: Mon - Fri (5 days/wk.) 8 AM to 4:30 PM (32-40 hrs. weekly guaranteed)
Mileage: Reimbursement provided (from first patient to last patient)
On Call/ Weekend: No


Job Responsibilities:

1. Perform home health PT evaluations, reassessments, and Start of Care (SOC) visits.
2. Develop and implement individualized treatment plans based on patient needs and physician orders.
3. Provide skilled physical therapy treatment visits when required.
4. Complete accurate and timely clinical documentation in HCHB.
5. Educate patients and caregivers on treatment plans, safety, mobility, and home exercise programs.
6. Assess patient progress and adjust treatment interventions accordingly.
7. Collaborate with interdisciplinary care teams to ensure continuity of care.
8. Maintain compliance with agency policies, documentation standards, and home health regulations.

Qualifications:

• Current Physical Therapy license in the state of practice.
• Home Health experience required .
• Experience with Homecare Homebase (HCHB) required .
• Current CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.
